On Your First Visit
You should bring the following items to your first appointment:
- Dental insurance information
- Any names of dental specialists you are currently seeing or have seen before (periodontist, orthodonist etc)
- A list of all prescription and non-prescription medications you are currently taking or have taken recently.

Financial Policy
As a service to you, our office will submit your changes to your insurance company if you supply us with complete insurance information. Your insurance plan is a contract between your employer and the insurance company. All patients are financially responsible for their accounts. We will estimate your benefits based on the information your insurance company provides. Insurance companies have different usual and customary fee levels for different areas. No insurance plan covers all dental expenses. It is the patient’s responsibility to pay any deductible amount, co-insurance (co-pay) at the time of service. All patients are responsible for monitoring their maximum annual benefits, and plan exclusions. After insurance payment is received, any other balance in regards to treatment is the patient’s responsibility.
Our office accepts cash, check, Visa, MasterCard, Discover and American Express. If a financing option is necessary, Please ask us about immediate credit with Care Credit.
There will be a $35.00 charge for each check returned because of insufficient funds.
The parent who requests treatment and is present with minor at the time of their appointment if considered the responsible party for all fees for services rendered.
Appointment Policy
We make every effort to value your time and we schedule your appointment time just for you. We truly appreciate your courtesy of giving us 48 hours notice (2 full business days) if you have a conflict with your appointment and need to schedule a different day or time. This allows us enough time to accommodate another patient who is in need of dental treatment. We are committed to your oral health. Keeping you scheduled appointments allows us to be partners in your dental care.
The large majority of our patients consistently show up for their appointments as planned and we are very appreciative. Unfortunately, there are some that do not show or cancel late for appointments that they schedule without the ample notification. Therefore, at the discretion of our practice, multiple missed appointments and late cancels without 2 full business days notice will results in dismissal from our practice. We will be happy to forward any records to a dentist whose hours may better suit your schedule.
